Take this paper along with the following documents to Al Baraha Hospital to get Birth Certificate and attestation.\u00a0+\u00a0To apply birth certificate through online; call<\/p>\n<p>Qusais\u00a0(Al Nahda-2): 04-239 1302,\u00a0055 273 2295,\u00a0055-345 7829<\/p>\n<p><b>Documents Required for Birth Certificate:<\/b><\/p>\n<p>+ Birth Notification issued by the hospital<br \/>\n+ Original &amp; copy of parents&#8217; passports with residence permits<br \/>\n+ Attested marriage certificate<\/p>\n<p><b>Time:<\/b>\u00a02-3 working days<\/p>\n<p><b>Procedure:<\/b><br \/>\n+ Some public hospitals such as Dubai Hospital (04-7075805), Rashid Hospital (04-3371111) and Al Wasl Hospital (04-3241111) have a counter which will issue the Arabic birth certificate. Go there and hand over the documents and receive the Arabic birth certificate immediately.<\/p>\n<p>+ For an English birth certificate, go to Preventive Medicine Department at Al Baraha Hospital (also known as Kuwaiti Hospital 04-2710000).<\/p>\n<p>+ Get an application form from the Birth Certificate Office. A translator will type your application at the hospital. You can get it attested at the Ministry of Health Counter at Al Baraha Hospital itself.<\/p>\n<p>+ Take the documents to the Ministry of Foreign Affairs for attestation and Pay\u00a0<b>Dh155<\/b>.<\/p>\n<p><b>Fees:<\/b><br \/>\nArabic birth certificate\u00a0<b>Dh50<\/b><br \/>\nTranslation fee\u00a0<b>Dh15<\/b><br \/>\nEnglish birth certificate\u00a0<b>Dh50<\/b><br \/>\nAttestation fee (MoH)\u00a0<b>Dh20<\/b><br \/>\nAttestation fee (Ministry of Foreign Affairs)\u00a0<b>Dh155<\/b><\/p>\n<p>After the birth of your child, do not rush to get the birth certificate. Wait till your wife and child have been discharged from the hospital and you have had a few days&#8217; time to finalize a name for your child, before you proceed to get the birth certificate. However, do keep in mind that the maximum period to register your child&#8217;s birth is\u00a0<b>30<\/b>\u00a0days.<\/p>\n<p>You must have a valid health card in order to deliver in a public hospital.
